Back in the day when the local Sbux became THE spot for my high school crew to hang out, my go-to order was always a grande iced caramel macchiato. In fact, I credit this salty-sweet dream with being my gateway to the wide world of coffee goodness! No wonder this is such a popular drink!

The Italian word macchiato means “marked,” which is fitting for this layered drink. In the classic (i.e. plain) version of a macchiato, espresso is poured on top of the steamed or iced milk, marking the white surface with inky brown. If it’s served in a clear glass, you can see the lovely layers, with the coffee and caramel drizzle slowly melding into the cold milk.
To make your own iced caramel macchiato coffee shop style, we add a few more layers of sweet yumminess. It starts with a base of sweet, aromatic vanilla syrup, a layer of ice to act as a barrier, a generous pour of creamy milk, the requisite espresso shot topper, and, as a final hurrah, a sticky sweet drizzle of caramel sauce. It is, in short, the only kind of “ombre” this naturally brunette gal gives a hoot about. 😂
What does it taste like?
What I love about this caramel macchiato iced coffee drink is the layers. Unless you give it a stir with your straw, you’ll be able to taste each individually. I like to take my first sip as I’m putting the straw into my cup so I can slurp my way though like a flavor archaeologist.
If you use this method, the first flavor to hit your tongue will be bold espresso mingling with sticky-sweet caramel — a lovely balance of bold and slightly bitter with burnt sugar undertones. As the straw continues its dive, the natural, mild, creamy sweetness of milk joins the party, softening the flavor of the coffee.
Finally, when you hit the bottom of your cup, you’ll get a potent hit of vanilla. These distinctive, stackable flavors are delicious on their own (which is why I don’t stir mine if I can help it!). But, the sum is so much more than the individual parts.

- Vanilla Syrup - Sure, you can buy a bottle from Starbucks or your local grocery store, but why spend all that money? My 3-ingredient homemade iced version takes just 5 minutes of active work, is super cheap, and making your own vanilla syrup tastes just as good (if not better) than the store-bought stuff.
- Whole Milk - Fun fact: if you order an iced latte, macchiato, or other milk-based drink from Starbucks, the default milk option is whole milk unless otherwise specified in the name. So, if you wanna make this iced caramel macchiato recipe Starbucks-style, use whole milk.
- Espresso - You can technically use any roast or variety of coffee beans so long as they’re ground super-fine and extracted under high pressure. I recommend 2 shots of espresso made with your favorite coffee beans for this recipe!
- Caramel Sauce - A homemade recipe is forthcoming on the site, but for now, this store-bought version is tasty and affordable. Feel free to swap in whatever caramel sauce you like!
- Ice - If possible, opt for ice made with filtered water for the best taste.
Substitutions
When you make an iced caramel macchiato at home, you get to call all the shots — without having to pay any silly upcharges! Here are some easy ingredient swaps to try:
- Vanilla Syrup - While Starbucks uses vanilla, you can double down on the caramel flavor with caramel syrup instead. You can also go non-traditional and use brown sugar syrup or sugar cookie syrup if you’d like!
- Whole Milk - You are welcome to make a low-fat or nonfat iced caramel macchiato by swapping in lower-fat dairy milk. Or, use soy milk, oat milk, cashew milk, or any favorite milk of choice! Read on under “recipe success tips” for help choosing.
- Espresso - Feel free to swap in cold brew coffee or strong coffee brewed in a coffee pot that has been chilled. You can also use instant espresso, too! You can also use decaf beans if you prefer!

Step 1: Pour the vanilla syrup into a tall, empty glass.
Step 2: Top the syrup with ice. This will help keep your layers distinct from one another!
Step 3: Pour your milk of choice over the ice.
Step 4: Slowly pour the espresso over the milk. If you want very defined layers, consider using a black & tan spoon.
Step 5: Drizzle the caramel sauce over the top in a hashed pattern. Drink up!

FAQs
The big thing to remember about a macchiato is that, if made properly, it should have defined layers. The only real layer to see in a latte is the foamy steamed milk topper. But, aside from that, iced caramel lattes and macchiatos are quite similar. Both are sweetened with coffee syrup (though a caramel latte has caramel syrup and caramel sauce while a caramel macchiato has vanilla syrup and caramel sauce), and the approximate ratio of espresso to milk is the same.
I can’t speak for all recipes, but this Starbucks copycat caramel macchiato recipe is made with homemade vanilla syrup, ice, whole milk, espresso, and caramel sauce — just the way they make it at Starbucks. It does have caffeine (though you can reduce it by using decaf espresso) and dairy (though you can swap in barista blend plant milk). And, unless there is an issue with cross-contamination, it is also gluten-free.
I’d consider it sweet without being too sweet. That said, taste is subjective. Feel free to add more or less syrup and sauce to make it more or less sweet. You can also make it more or less strong by adjusting the espresso and milk ratio to your liking.
You can if you want all the flavors to blend, but keeping the layers separate is part of this drink’s charm.

this is like my favorite drink ever!! best recipe for cold coffee! it is super easy and if you just want an awesome cold coffee just ditch the caramel and you get perfect cold coffee. tip for the vanilla syrup: take a small mason jar/cup and put the sugar and vanilla in the jar and then pour some of the hot water you are using to make the coffee and shake in small circles till the sugar devolves and then pour however much you want into the drink. it is so easy and then you don't have to store it.
